Please furnish the following information:
Copy of high school diploma, GED or college degree
Copy of your National Registry certification (If you do not have a NR, you will need to apply for State Reciprocity)
Copy of your State Paramedic License
Copy of your BLS-HCP, ACLS and PALS card. Must be official AHA card. No substitutions.
Proof of successful completion of your most recent DOT—approved Paramedic course
Copy of driver’s license
A signed authorization form for background check

Step 2: A link for you to take a timed online multiple choice exam will be e-mailed to you. The exam will be available from Monday, November 2nd at 0900 until Friday, November 6th at 1700.
Step 3: The candidates with the top eight scores will be invited to a one day (0800-1700) practical testing and interview at our administrative building, 6116 Graye Lane, Caldwell, ID 83607, on Monday, November 16th.
Step 4: Successful candidates could be offered a conditional job offer based on ranking and passing a physical exam, pre-employment drug screen, and background check, until positions are filled.
